We developed a complex multi-part form using Ninja Forms. This form collected the information through six screens and then aggregated it together into the submission. The submission was saved in the backend of WordPress, collected together into an PDF, which the applicant must sign and mail. As the well, the PDF is saved into a document management system (Egnyte) and finally, all the 3-4 persons’ information included in the form are added to a CRM (Insight.ly) as leads. Emails are sent to administrators and to the application.
This form allowed the client to develop a working prototype for an application system. There are plans to extend it, so that it is more deeply integrated with the CRM. But for now, this form does exactly what the client needed it to do.
